The most common causes for the requester not getting email confirmation once the request is approved are:
1. Requester Email has not been selected on Form Workflow Plus Basic Setup tab
To select the Requester Email address check the box "Collect email addresses" on your Google Form.
2. Your G Suite domain does not have Gmail enabled. If Gmail is not enabled the reviewer will not be able to send the approval email back to requester.
