Why doesn't our workflow notification show proper values?

1/ Make sure all column headers on your spreadsheet are mapped correctly to your tags. If you change a column header it will no longer be correctly aligned to tags in your template. 

Use the Available tags drop down menu to map your tags

2/ If columns have exactly the same name, the Form Workflow Request for approval email to reviewer will not display all values. Each column header must have a unique name
